Who we are:

Excelsior Wellness is a nonprofit healthcare system. We are comprised of four integrated behavioral health, wellness, and primary care organizations who provide comprehensive wraparound services to meet the needs of our patients and our community.

Our philosophy:
Our approach connects behavioral health, primary care, and social and human services to build a foundation of wellness. Our holistic philosophy means we see, honor, and care for the whole person and meet them where they are physically, emotionally, mentally, intellectually, financially, socially, and spiritually. In every aspect of care, we seek to create partnerships for wellness and recovery. We put patients at the center of care and aim to be trusted partners on their wellness journey.

• Excelsior Family Medicine – Person-Centered Primary Care and Foster Care Center of Excellence
• Excelsior Wellness Center – Community Behavioral Health Including Wraparound Care for Children, Young Adults, and Their Families
• Excelsior Integrated Care Center – Higher Level of Care Including Bridge Housing for Young Adults
