Horizon Hospice

Horizon Hospice provides services that offer palliative care  for patients with incurable illnesses. Our goal is that noone would die alone, outside their own home, or in pain.  We officer a holistic approach to end of life care provided by a team of individuals including Dr, nurses, bath aid, social workers, chaplains and volunteers.
